Failures or near misses at a facility or during construction activities benefit from a root cause analysis with the objective of providing the best possible understanding of the causes of the failure and how to minimize or mitigate them. We are trained and experienced in the ability to perform a root cause investigation, utilizing various analysis approaches and techniques that include clearly defining the problems, gathering evidence, classifying causes and identifying recommended corrective actions.
